Freestyle Promise/Diabetes supply line

    Has anyone been successful using the Freestyle Promise program through a Diabetes supply line? We have Providence health insurance and our plan dictates that we MUST get our supplies through their diabetes supply line and not a pharmacy. When I asked them about the Freestyle Promise program, I was told they could not process that for me since they aren't a pharmacy.

    I was just double-checking to see if that's everyone's experience.

    Call the Freestyle Promise program and request for a form that allows you to get copay reimbursement manually. You have to mail the form with a copy of the receipt/invoice from your pharmacy. Reimbursement takes a couple of weeks.
