Hi everyone, I was wondering if you could share your sliding scale for rapid acting insulins. Autumn is on Humalog her scale is
    70 – 200 no insulin
    201 – 250 1 units
    251 – 300 1½ units
    301 – 350 2 units
    351 – 400 2½ units
    401 – 450 3 units
    451 – 500 3½ units
    > 501 4 units
We are trying to come up with a scale to eliminate the 1/2 units for school time. She is not yet dosed for food. And we are working with our dr. but I wanted to see what you all used. The "pink panther" book says that our goal should be 70 - 180, so that is something we are trying to change. Thanks for your input.

P.S. for those of you that have helped me out before, we had to go back to NPH in a.m. and lantus in p.m. Her numbers started running in the 400's at lunch. So far no lows in the a.m. :)

Just bumping to the top for you. We don't use a sliding scale and I don't think many others here do either, but hopefully someone can help you!

Usually the sliding scale is just used in the first months after diagnosis. At this point, it is worth working with your medical team to learn to count carbs and cover them with insulin using your child's insulin:carb ratio.

It sounds complicated, but isn't really and will allow you to be more flexible with meals and snacks.

We use a sliding scale of sorts when we give Danielle a shot because of a failed site. She also used it daily up until she was on the pump for any highs along with counting her carbs. However, her scale is WAY different. Her scale right now is 1u of insulin for ever 20 above 150 (I think...she knows that target better than I do right now).

Ours is the same as Decota's - that is our "correction dose", then we count carbs and add that dose to the correction. Luckily we are seriously honeymooning and I can't remember the last time I had to correct. (knock on wood)
