Connor uses a Novolog Novopen Jr. for fast-acting insulin. His new-ish school nurse sent this home today. What do you think? What is your reaction? Have you ever seen a Novolog pen cartridge with a "stopper that balloons"? I just sent the manufacturer instructions to her because she didn't know about priming the pen before giving an injection. Now I don't know what to make of this new cartridge/pen issue. Any thoughts? Thanks!

That is a weird one. Sounds like you may have to go in and do a demostration or at least see what she is talking about - I know this isn't really a possibility if you use the pen correctly but could she be trying to put air into it like you do a regular vial of insulin????

There is another child with type-1 diabetes at Connor's school who uses a syringe and vial of insulin.

I remember seeing this before -I think it happened when we would prime without a pen needle on.

![]() That would make sense from what I've seen when I'm in the clinic. Thank you, Lee. I think that is what's going on.

That's what happens if you dial up and press down without a needle on, or with a needle that's bent on the bottom so it's not puncturing the top of the penfil. It means there's a lot of pressure inside.
Screw a needle on, a drop of insulin will come out right away, but it will normalize. No worries. I used to keep mine pressurized like this on purpose on the theory that it would get rid of the air. I'm not sure it worked. My brothers and other people playing with the pen have also done this to it, and also once or twice with a needle where the part that's supposed to screw onto the penfil apparently bent and hadn't gone in.

I'd really like the school nurse to follow the manufacturer instruction manual for using the NovoPen. How do I approach this topic again without alienating her? Sigh...

Somebody on the teentalk forum wrote (years ago now) about a museum security guard who forced her cartridge to explode like that too. In her case the guard ruined the whole cartridge.

sounds like air was added.. or it was dialed and 'delievered' without a pen needle.

I guess the only way I'll know for sure what is going on is to go back to school again tomorrow and have her show me how she is priming the pen and changing cartridges. Shouldn't a school nurse (an RN) know how to do this stuff? My stomach is already in knots thinking about having to go back to the clinic to have this conversation - again. I just don't want to keep sending in new cartridges of insulin and having them wasted from user error. If she doesn't know how to change a cartridge, she can refer to the manual, call her supervisor or our endo's office, or call me and I'll go over and help. I just get a mangled cartridge after school hours with a note to send a new one tomorrow. Fine, if I didn't just get one sent back home that was destroyed by the nurse. The cartridges do not come out of the blister pack like that, so I'm agitated by the waste. Grrr...
