Survey about creating a Family Resoucre Center

I've been contacted by a mom who is exploring creating a Family Resource Center for families who have kids with type 1 diabetes. She has a brief online survey that will help her. If you have a child with type 1 and have a couple of minutes to spare, please help her out.
