Default My 4 AM Questions

Fun night last night which has left me with a couple of questions.

Ian woke up about 3AM last night and vomited. He checked his ketones at which time I discovered the strips were about 1 month expired. Ketones were negative. I'm assuming the negative result was wrong. Anyone have experience using expired strips?

His BG was 345. We waited an hour and rechecked and he was 300, which got me to thinking what do you consider a good number of points to drop in an hour? A drop too little may indicate a set change is in order, and a drop too much will make me worry about going low. So what do you think?

I wouldn't necessarily assume that the strips were incorrect because they were expired.
Jordan doesn't get ketones easily, so he could have been that high and had no ketones, it happens with us sometimes too!
I'm not sure about the amount to drop in an hour though, I'll be watching to see what others think.

How is he feeling now? I hope he is feeling good!

1. I'd trust the strips. It's not like they go bad the day after the expiration date, and a month is just not that long expired. I'm sure we've used expired ketone strips before, and I don't worry about it too much until they're REALLY expired.

2. I assume that you corrected the 345? I'd be fine with a drop to 300 in an hour. We see the biggest drop around 1.5-2 hours, but with a 45 point drop at hour 1, I'd feel confident that the correction was working. (ETA: With a big correction like that one would be, we see BG going down for much longer than a meal bolus or a "normal" sized correction. So we'd still be seeing a BG decrease at hour 4-4.5. One hour is kind of the "tipping point" where we see corrections starting to work. Checking at even hour 1.5 tells me a lot more about how well the correction is actually going to work.)

I do use expired blood ketone strips since I use them so infrequently. Did he use blood or urine? I haven't used the urine strips since being in the hospital, so I don't know how those hold up.

Going from 345 to 300 in an hour isn't something I'd consider a drop at all, actually. I'd expect something at least in the mid-200s. While I wouldn't want a 100 point drop from the 200s, I like to see it coming from 300s. (ETA: I'll add that my daughter is in puberty, and already insulin resistant, and that's why I'd consider 45 points hardly anything for her.)

He is feeling fine now, thanks. We both decided to stay home from school since we were up from 3AM on.

Yes, we corrected for the 345 and changed the set eventually this morning just to be safe.

Ian does not get ketones easily either, but the couple of times he's had them he's always vomited so I just kind of suspected he had them.

I've used Urine strips after they've been open nearly a year, as long as the expiriy date is ok, and the lid is on tight. And they've always been correct for me. I only check when I'm trying to lose weight, so if I'm eating no carbs, they turn those nice shades of purple! I have checked them with blood ketones for accuracy!
