View Full Version : Spliting Lantus into 2 doses

I remember reading on the forum when Devin was first diagnosed about some people have to split their child's Lantus dosage up into two different shots.
How does that work and why would you need to do this?
I didn't pay much attention to it then because Devin wasn't on Lantus.

When Scott was on lantus we started out giving a single shot at night. We found that with him there was a peak during the night and this was causing night lows. We changed to a morning dose so that the peak was during the day and we could manage it better. We then found that the duration of the insulin for Scott was not lasting the 24 hours so we needed to have a top-up in the evening. If I remember correctly prior to starting the pump he was on about 28 units of lantus in the morning and 7 or 8 units at night. He is also a very active teenager, not a smaller child.

As Scotty wrote, I recall there being two main reasons to split Lantus, to reduce the pike some get and to ensure the 24 hours is covered as it seems to fade before 24 hours for some.

We split Reagan's Lantus dose because hers wasn't lasting the full 24 hours.
She was going really high a few hours before she was due her next dose.
So we split it and started giving it to her at 8am and 8pm. Our Endo didn't like it, but it really did work for us until we started pumping.

Exactly the same reasons we split the Lantus (as well as a similar reaction from the endo, who ultimately said, "Well, I can't argue with the a1c!").
