My son is being transported to my wife's job from school. I was trying to figure out what type of info I should give the van drive.

Ask the school what they require for kids riding a school bus with a medical situation. I include the following for my child:
Picture of child (in case of a sub)
Laminated description of condition along with symptoms of low bg
Juice boxes
Glucose tabs
Card with my/husbands cell phone #
Our bus driver keeps all this in a ziploc bag up front with her.
Depending on the age of your child he/she may carry a meter on them at all times. If not, I wouldn't bother. Our bus driver knows when in doubt to treat, and in a real emergency, call 911. Most EMT's will not test with a childs meter anyway nor will they administer glucagon.

Give a list of symptoms of low blood sugar.
Child appears sweaty
is tired, falling asleep
Child complains of feeling shakey
Child complains of vision going blurry
Child says "I feel low."
NEEDED ON VAN
Glucose tabs
Juice
peanut butter and crackers
emergency number to contact you and your wife
the common sense to call 911, pull over and wait for the ambulance.
