Ok guys. Devin's old Endo had us checking him at 4am (which I never understood why...I thought it should be 2-3 but anyway) but his new Endo changed it to 2am (which is about to KILL me I am so tired but anyway) I have a question about what is considered low for 2am??

Night before last he was 56 so I knew I had to treat that. Last night it was 88. I plan to call the Endo this morning but at what #'s should we treat him at 2am????

PS his insulin regimen right now is 1 3/4 units of Lantus at 7:30pm (yes 3/4 is odd but 1 1/2 was not enough and 2 was too much) and then 1 unit of Novolog at each meal (but today we will probably be trying 1/2 unit of novolog at each meal because he has been going low after each meal even after eating the 45 carbs. YES HE IS HONEYMOONING....LOL

I think "how low is too low" is going to be based on what the trend is showing that he does between 3am and wakeup. We used to drop during that time, so we would treat an 88. Now, we stay smack dab where we are between 3am and 7am, so I don't usually treat.

I would think with his age that you would treat that number as a low.

When my daughter was younger, her endo did not like her number to drop below 100-120 at bedtime or during the night.

I would check with his doctor to see what number he wants you to treat at.

I would treat that number - but just with 4 carbs...we should be between 100-200 for bed, but at 2am, she still has a lot of sleep left in her. So yep, unless she woke up high the next day, I would plan on treating anything under 100 (I actually do unde r120 - I worry!)

Our Endo wants us between 150-180 at the 2/3am mark.

I agree with Nancy. 88 is a great number, if the midnight number was also in the 80's or low 90's. We try our best to look at the trend, and generally treat if BG has changed more than 20 - 30 pts in the midnight - 2/3 am time frame.

It helps to know how each child's BG usually moves during the night -- something you can only learn through frequent blood checks.

Well of course I was questioning not treating him thats why I made the post waiting till the dr was in so I could call him. Devin usually wakes about 6:30am no matter what time they go to bed the night before and I FINALLY had to go wake him at 7:45am. I didn't realize the time and FREAKED out. My 11 year old nephew is spending the day (to learn to take care of Devin) so we ran in there and Devin just sat up in the bed and laughed at us. We changed diapers and did his BG and he was at 79 which is really close to what he normall is upon waking (usually 80-90) but I am going to talk to his dr today and find out exactly what he wants me treating.

I agree that you need to check again at 5:00ish to see how he trends from the 88. If he stays steady, fine. My dd can be around 100 at 2:00 a.m. and 60 at 5:30, so it just depends. Sleep - who needs it :->???

Given his age and insulin regimen, I would treat it (though with less carbs than the low in the 50's, in order to prevent a rebound high).

My son is 12 and on a pump...in our case I might choose to let that 88 go if I am confident his basal rates are set properly.

Sounds to me like you handled this one magnificently! 88 at 2 am and 79 upon waking - nice and stable and it doesn't get much better than that..

Other posters were quite right - if you know that your child is relatively stable or only dropping slightly through the night, then 88 is great number. If things are less predictable, then you may want to correct with a few carbs.

I should note that our DD was also 88 at 1:30 am. I stayed the course and she was 94 at 3 am, and 112 at 8 am. Yippee!
