Hi,
I am trying to figure how to correct high BS with Caitlin. As many of you know she has been sick with stomach virus (vomitting only). Thank God, she is getting better and better, but her appetite is still not the best. Tonight before dinner her BS was 366:eek:. She did not eat well at lunch and I only gave her 1/2 unit to cover what she did eat. So after checking her BS tonight, she wanted to eat so I gave her 2 units of Novolog and then she decided that she really wasn't hungry. Eating, but eating slowly.
What I am trying to figure out is how do I correct a high BS (if she does not eat)?
I am kind of nervous now, because I am concerned that she might drop too low in the night
Help!!!!!!!
I was trying to use the 1800 rule, but it really does not make sense being that her total daily insulin dose is so low.
Daily Insulin Dose:
Lantus 2.5 units
Novolog 1.5 units (breakfast), 1 unit (lunch), 1 (dinner)
***This does not include any correction for BS out of her target range 110-180

When Phoenix is running high while sick or just running high period I give mini corrections, the smallest I can give is 1/2u so if he is 421 and ate just a tiny bit I will give maybe 1u novolog, recheck 3-4 hours later for another correction if nessesary, sometimes I am up all night doing this but I dont seem to get lows doing it that way. I get lows when he is healthy and eating me out of house and home :D

We arrived at 1:200 as a correction factor by doing a bunch of mini-corrections (1/2u) when 300+ and observing the results.
Of course, last night was an exception, he was 270 barely dropped with a correction, but it's hard to measure those small doses.
The 1800 rule doesn't really seem to apply to us yet, since Alex doesn't have a consistent TDD per day.

If he is high and it isn't at a meal or snack time; then we try not to give insulin for highs, because then you can get into that pendulum swing so easily. We try and bring down highs, with broth with a high salt content such as with that packet that comes in ramen noodles-we just don't make the noodles, or with plain salt water, with plenty of other fluids too. For Jacob if he is in the 300's it usually takes 16 oz of water with high salt content to bring him down to the 100's in about an hour and a half. Then he doesn't have that extreme low that can sometimes happen.
Anyways, that is what we do.

Hey, sorry to hear things are a challenge just now.
What the 1800 rule tells you is that using Novolog to bring down BG is going to be tricky, because 1800/6=300mg/dl... so just 1 unit novolog will bring her down hard and fast if she's not eating. My sense is to give 1 extra Lantus and BG will come down slowly but steadily, and you'll be able to sleep. If still high 12 hours later, give 1 more. This should get you where you want to go, more gently.

Thanks for the advice. Makes sense. Caitie was eating so well and suddenly she decides that she will or will not eat (Even if I prepare her favorite foods)

Have you been testing for keytones? If she's sick, especially with the flu...I think this is a good endo question. Especially when she's so young, do you have a correction ratio? Don't use this, but for example...Seth's is 1/2 a unit for every 50 over. Also, for now, you might want to give her shot after she eats, in case she decides she's not hungry..

BS-150/120
I have been dosing the insulin maybe 10 minutes or so after she starts eating.
I think that it just hit me how much really 1 unit of Novolog can lower the BS.

Care to explain more, or send pointers to more info? I've never heard of this.

My doctor recommends giving 1 extra unit from their regular dosages.
