View Full Version : When do you give Lantus??

We are giving Zach his Lantus at bedtime (around 7:30 pm). We started with 2 units and the the endo upped it to 3. We are having a very hard time with him bottoming out at the 3 am BG checks (60-80 range). Do you have any idea how hard it is to wake a 2 year old up and try to get him to eat or drink something? The worst part is that I have to get him up at 5:30 so I can be at work by 7. I can't believe there couldn't be a better time arrangement for meds. Would he be able to take the Lantus when he wakes up? We are waiting to hear back from the endo. BTW:His Humalog ratio is 1 unit to 15 g And we are having some issues with highs during the day. ???????

We were giving my son his lantus at night and he was also going low in the morning. I agree it is almost impossible to make a 2 yr old eat or drink. We now give him the lantus in the morning and it has helped tremendoulsy. We have only been diagnosed for 2 weeks so we are still up and down but moving the lantus helped.

We're just starting our second week so I guess I am just being a little impatient. I just want to do the best I can as soon as I can for him.

We have a 19 month old and we give 4 units Lantus at 7:15am along with a shot of 7 units NPH. I dread having her go low in the night b/c it is close to impossible to get her to drink (let alone eat) once she is alseep. When we have a low, I prepare her a sippy cup full of fat milk and then I sit with her and watch tv. Tough part is getting her to go back to bed after the interruption.

William is on the pump now but when we did lantus (he was 2 yrs old at the time) we always did a morning shot. I've read that lantus can have a bit of a peak.

We had no problems doing this and his lantus seemed to last just fine until the next morning's shot.

but lantus usually can peak around 5-6 hours ( ie may cause lows..) we use dto use lantus and novalog and the endo right away said its up to us but he suggest am's bcaues of the lowsduring day if any ( due to eating) vs at night causes the lows and no food on board to catch.. the ams worked great.. In a year we only had about 1 maybe 2 unexplained lows..

WEll my daughter was just diagnosed and our endo said he doesnt recommend lantus to kids so young because of the instability of it, my husband wanted them to try it. But so far it is just a week and we are going with what the dr's say. WE are having lots of highs right now. Now she is on NPH and diluted Humalog. She is 17 months old, we are trying to get the pump ASAP!

My son was 3 when dx'd and he was on NPH and Regular. He stayed on NPH until he was seven. Lantus helped the stability A LOT. I don't know about Lantus with young children, but I think NPH is not particularly helpful in managing the sugars.

I hope you can get the pump soon. It is hard to give a toddler tiny amounts of insulin.

I presume the Endo upped the Lantus dose because Zach was having high numbers? I found I had to increase Rachels Lantus dose in half or even 1/4 units to prevent lows. If you think about it you have increased his basal insulin by 50% - thats a huge jump. Endos and specialists don't like thinking in smaller than unit amounts, but for us parents of small children its a necessity.

We used to give Rachel her Lantus at 9.30pm - it was a convenient time because if we were out for the evening then we were usually home by then, and I liked doing it while Rachel was sleeping to reduce the number of shots she knew about. It worked really well for us , but we didn't get the peak some people seem to have.:cwds:

Before pumping Austin got his lantus at 9pm. when he decided he wanted to go to bed earlier we asked the endo if we could push it to 8 she didn't have a problem with it.

You just might need to move in smaller steps toward your desired time.(ie from 8-6pm, 6pm-4pm, in two hour increments for a while so that it is not so drastic of a change)

I split my Lantus almost 50/50 at morning and at supper. Morning is about 3 units more than supper. I did this because I was finding that my bs was drifting up and not lasting the whole 24 hours. At first I was just increasing my dose (which was in the morning) because my numbers were high in the morning, but was finding that I had too much insulin on board during the day. After I split my dose I actually reduced it in total by about 5 units.

This being said, I am an adult and on way more than a small child. However I have heard of some children splitting their dose as well.

Lantus doesn't really have a peak so to say, but it does have a "kick in" factor within a few hours of taking it. So to speak, it takes a few hours to reach its maximum, but maintains that level, so if you are splitting the dose it doesn't really wear off at any point.

Just my thoughts on the matter.

Hunter was getting his Lantus b4 bed at dx. About two weeks later, we changed it to morning as well because he was having some middle of the night lows. This way, you can see his mood and check for lows during the day instead of worrying all night if he's going to get too low. Now, we had to start giving 1/2 u. in am and 1/2 u. in pm because he was going high in the middle of the night and the Lantus was wearing off. His daytime numbers are good, that's why we didn't just simply increase his am does.

We were also having lows with my 9yr old daughter. She is on 9 units. I give it to her with her bedtime snack, and we actually adjust her carb ratio just at bedtime snack. Through the day she is on a 8:1 ratio, but now with the Lantus at bed time, with that snack we give her a 15:1 ratio, and that seemed to do the trick, with her waking up within the 120-150 range, any lower gets me a bit nervous:o HTH
