Does anyone have a user friendly instruction form that has information on how to care for our kids. I want something that has pump info as well...Like how to give a bolus. I noticed that someone on the CGM forum had a form that even had pics of the pump keys for instruction.

I always rewrite this for every caregiver, and inevitably leave something out. I would like to get something I could laminate and give to anyone who takes care of my Rachel... I hate to reinvent the wheel if someone out there has a perfect one!!!

I will try to attach what I use. I have printed all these out and put them in a binder for who ever is babysitting. It is usually grandma or grandpa...but we also keep on in our travel bag.

I basically took instructions from the pump manual but condenced it.

I have attached a little cheat sheet that we use. It is in Ross's pump pouch at all times so if someone at daycare needs a reminder of what to do or a grandparent needs a reminder, it is always there. Hope it help. It only applies though to those of you who are using the minimed pump. Sorry!
