What are the typical criteria for kids to be put on a continuous glucose monitor as well as an insulin pump?

I don't think there is a typical set of rules for this. Different endos have different opinions about this. Some endos will send kids home from dx with a pump, others suggest waiting until after honeymoon is over. Others want to wait a set period of time to allow the kids and parents a chance to learn how to manage the disease first without having to learn the pump. Ultimately I think it is up to the kids and parents to decide the right time (as long as they have done their research). If the Dr. doesn't agree then I say fight for it or find a new endo. The important thing is making sure that the child is ready.
I will say that having accurate carb counting skill and using the MDI regimen that combines basal insulin (Lantus/Levemir) with bolusing for meals (Novalog/Humalog/Apidra) will prepare a family better for the pump regimen.

MDI = Multiple Daily Injections, (usually 3+ injections per day, minimum)

Basically it depends on the doc and on the parents. There are lots of toddlers on this forum who pump and there are some on CGMs as well. We started pumping 6 weeks after leaving the hospital and hope to start a CGM in days. We also had to switch docs because we wanted the pump but the first doc who we ended up with at dx was not so progressive, more old school type.
I wish we started pumping right away.
