In the early stages of Type 2 (or 1.5) when the body is producing considerable amounts of insulin, just needs a little help beyond oral medications, which approach is better:

Giving only basal insulin to spear the capacity of the body to deal with post meal highs or giving boluses to help fight off those highs.

It would seem to me that the body is better at fighting highes than boluses are, as long as the body is able to produce enough insulin. Also it is easier and more conveniet to inject basal insulin 1-2x a day than carb counting and bolusing for every meal. However, I don't know if the capacity of the body can be "speared" like this for postmeal highs.

I myself have learned this the hard way in my current honeymoon. Type 1.5 so they say. I was taking 5u of Levemir/day and would drop about 60-150pts at night. I had to go to bed around 230's to be safe at night so I always had to have a snack. As I saw the post meal numbers climb more and more over time, I realized that I should try the other way around first, since my pancreas could not keep up WHEN I ATE. My numbers would ALWAYS go down down down very fast when fasting on the Levemir which isnt right as far as a properly set basal. So, after 9months I was finally motivated to figure it out myself since they dont give us adults any education. Turns out I stopped the Levemire in December and have had awesome results. If I want to skip a meal, I can and do, and my pancreas does a great job lowering my BS to about 80 everytime. Does not go lower or higher. I dont even need a bolus for 5 carbs. I can eat up to 20c on a VERY active day without needing a bolus!! Its great!! Sure, my BS might spike a bit, but it goes back down itself, a true indicator that your body is keeping up with your basal needs. I am sorry I waited so long to try it!!
