So we have had our 504 in place for a year and have adjusted it this year. Hunter is getting a pump this month and I have to know what to put in there RE: the pump. All advice welcome!!!!

Hi Linda, Tyler is older so his 504 info wouldn't be the same. There is a parent that I was speaking to that has a child about your age she has on going pump training as mandetory. How to do infusion sites if neccesary, trouble shooting etc. Their pump reps come to the school to keep the staff up on training.

If you look at the DREDF 504 they have a list of what the child may or may not need assistance with this should be in there. Also storage of pump if the child chooses not to wear it in gym or recess, (who's responsible for safe keeping). I put also that if they will be working with high power magnets in science (Ty's in junior high) that Tyler is to remove his pump and teacher is responsible for safe keeping.

Also the new protocol requires a change in the individualized health plan or DMMP whatever term you use for treatement if the child has a number above whatever your protocol is for tx with shots and new infusion site changes ketone checks etc. The list in the 504 for capability also needs to go into his DMMP and be signed by your doc. You could do it as an attachment if you would preffer.

Hope this helps. There is probably more to it for a young one but this is what I did.

Schs., Complaint No. 11-00-1008, 34 IDELR 43 (OCR 2003) (school agreed to train school personnel in the use of pump and also have an individual trained to operate the pump accompany the student to school-sponsered events off campus.

The CWD website has a bunch of sample 504 plans and those for pumpers are indicated: http://www.childrenwithdiabetes.com/504/

You might cruise through those and see what references are made to the pump.
