View Full Version : changing shot time?

We have had a few lows over the last few nights. I talked with the hospital today and they suggested changing the time that I give her the Lantus shot. Right now she gets it at 830pm - right before bed.
They are wanting me to move it so she is getting it in the morning now. Thinking that she won't have the nightime lows.
Anyone have experience with this?
Did it help? We have all day to decide, just looking for some input.

Lantus becomes fully effective around 4-5 hours after the shot and less so 22-23 hours after the injection.
My son had the shot at dinner and then bedtime to better time when he would be high.
If your daughter's basal insulin needs are not high during the middle of the night, changing the timing would work.
The best control my son had was when he had two shots of Lantus -- at bedtime and in the morning. He has higher basal needs in the daytime than at night.

We moved the lantus to the AM for exactly that same reason, with much success. (Curtis was 4 at the time)
After moving to morning, and adjusting, we had very stable overnight numbers.
Try it, see if it works. You can always try something else if it doesn't.

Colin gets his Lantus in the AM, and it has worked quite well. We also started in the AM about 6 months ago (he had just turned 3) and have been very pleased. He rarely wakes up low in the mioddle of the night anymore.

Hi we are no longer on lantus ( now pumping) , but when we where we used it in the am becuase lantus tends to have a slight peak at about 6 hours.. ie 8:30pm +6 hours// peak when sleeping and no food on board to cover.... in the am by the time it peaks it will be generally during or after lunch.. you have food on board to help cover anything that is going on adn prevent the lows.. also it gives you the opportunity to see when lantus needs to be adj much easier.. lantus is 24 hours so it will tend to start to ware off during the night... so if your child wakes up HIGH, it needs to be increased.. if you child tends to wake up low,, it needs to be decreased... IT works out great taking it in the am... at least for us

We moved Emma's Lantus from night to morning as well. It did help some with lows at night. Did they tell you how to do it? I don't remember exactly how we moved the time w/o messing her numbers up too much. Of course her numbers were pretty awful already, so one night of highs and extra novalog probably wasn't unusual.
Do you have any idea when she'll be getting her pump?

We use Lantus in the morning and I am not as concerned with her going low at night. However we still do night time checks if there has been more activity than ususal - at which time we usually give milk or, lately, a graham cracker.
We have never done Lantus at night so I do not have much to compare it to yet we did give NPH at night - and we had lots of lows. Thus, we switched to Lantus and NPH in the morning and have had a better go at things. I have a feeling that it will work out well for you all, especially since Regean is so young. Good luck on your decision :cwds:

that's very common for little ones to get Lantus in am. We atarted in pm when we got home from the hospital, but w/in 3 days we were switched to am. We also do a split dose not. He gets 1 u. in am and 1-1/2 u. in pm.

They told me to start backing it up by two hours until I get it where I want it.
So from 830pm - tonight we will do 630pm, then tomorrow 430pm...etc
Should take 6 days to get her moved the 12 hour difference.
I am excited to hear that this worked for so many with little ones. I just wonder why the hospital didn't suggest it earlier?
I am waiting to hear from insurance/Dr on the pump. We have submitted the app, found out that insurance is covering almost all of it plus 3 month supplies (WOOHOO!) Last week I was told that it should take a week or two and I will hear one way or the other. The only thing that will hold us back is that we haven't had our 3 month clinic appointment yet. I have talked to some poeple at the hospital and I am HOPING that they will ok us for it anyways.

Really? eervy 2 hours? We just did the change and rolled w/ it. I didn't notice a difference, but then again, it was 11 months ago. I couldn't even tell you what his BS should be at night 11 months ago. Reagan is little right? How much Lantus is she on?

she is 12 months old and on 2 units of Lantus.
She will get it tonight at 830pm, so if I just gave it to her in the morning at 830am, wouldn't that be too much in her system?

Check with your endo on exactly how to do it. We did the pm to am change when Carson was on Lantus. I remember it went very smoothly. I think we gave him 1/2 of his regular dose at night, then 1/2 the next morning and then the full dose the day after in the morning. That way he always had it in his system -- and it was better to chase the highs rather than have too much in the system and chase the lows.
Good luck!

We did _exactly_ what BrendaK did. Halfsies, and chase some highs.
Did it every time we went to Europe too, to handle the time change.
Moveback 2 hrs works too, just sounds like a PITA.

2 hour moveback is what nurse at endo office told me to do.
I like your way better, but will stick with hers just in case I need them to help with something.
it was my least favorite of the nurses there and I am really trying to give her some credit. If this works she may be back in good graces again.

Well we did it - gave her the Lantus at 630 tonight
Checked her at 830 and she was 260, down from 280 before dinner.
We will check a couple times tonight and I will let you know how it goes.
Should I expect anything out of the ordinary? I was a bit suprised that she was still up there, but ok that the bottom didn't drop out either!
