View Full Version : what happens when you split the lantus to day and night?

Im there. Tonight Im going to give Kat an extra 1 unit of Lantus at bedtime to see if that helps fight her highs that start at around 2 am and then just seem to last..... some of you split the dose right? what happens when you do that? do you see a pattern of peak times?

I never split our dose. But I really just wanted to ask if she liked the pod - she tried it out for a day or 2 right? The pump would REALLY help with those spikes because you could set the different basals...totally off topic - I know, but I remember you talking about the pump, so I thought I would mention it :o

Oh i wish :( Kat goes to the endo next month and when she does, we get to bring the omnipod paperwork with us and talk to the dr about getting the trial. i ordered the free trial pod and got the empty shell with no remote "thingy", so she could not use it. i think she would love it, and it would help her. she was very excited when i told her that if she got to wear it she would not have to take shots anymore. (she also likes the fact that nick jonas wears one). im working on convincing my husband that the pod would benefit her (i have tons of print ups on the pros and cons of pumps and pods) so we can begin our fight with tricare to get it.

I would go on ahead and try to split the does tonight - that way you have all weekend to chase any lows if they happen!

I know a few folks here have Tricare and the Pod, so hopefully it won't be a problem. Have you called them and asked about coverage for it? That way you have some oommph when you go and talk to the endo?

I remember when we were on MDI I would give 4 units in the AM and 4 units in the PM. Checking all night long BG. She basically maintained but her 7:00 a.m check would be in the higher range of 200. I was very frustrated with the MDI. I was always chasing a high and feeding the lows. When we gave all 8 units at night she would drop low every night around 2-4am Checks.

But I have to agree you won't get alot of those crazy high numbers in the middle of the night with the pump/pod. You are more in control and can change things around the time the peaks start occuring.

Good luck I will keep my fingers crossed for you with the pod.

So Im curious now about Lantus effectiveness and how its supposed to work. Technically its not supposed to peak but if splitting the dose is working for you then isnt it peaking? Or perhaps its just fading away too quickly?

Ours fadded away when we gave the full dose at night. So we split it, every kids different with the Lantus this is one reason we went on the pump to avoid the lows and peaks so we could maintain some sort of normal. What that is when I find it I will let you know..lol.. But seriously the pump has been the best thing for us with the control of the insulin and not the lantus. I was always chasing lows in the middle of the night with it.

We also split our lantus dose. Scott used to get about 2/3 in the morning and 1/3 at night. (I think it was about 27u and 8u) We began on lantus giving it at night, but we did see a peak during the night so we switched it to a morning shot. We soon realised that the lantus didn't last 24hrs with Scott and that is when we split the dose. Now that we are pumping we have the variable basal rates and this is working so much better for Scott.

This was exactly how we split ours too when we were on shots! It worked great. No peaks.:)

k. so if you split into 2/3 and 1/3, Kat would get 10/14 in the am and 4/14 at night. Ill start it like that tomorrow but for tonight, just 1 extra unit to "get her going". thanks for all the replies.

Let us know how you get on. We always waited 3 days between dosage changes with the lantus so we saw the full effect of the change before making yet another change. Things might be a little strange for the first few days after a change like this.

My son was on Lantus for about 5 years and is on a pump now. He had great contol on the split shot of Lantus. He received one shot at 10 pm and the other at 7 am -- the amounts were different.

Lantus doesn't peak like the fast acting insulins, but it has a plateau that is reached about 4 hours after the injection and then descends about 22 hours after the injection. There is a valley around the time the injection is given where the long acting insulin is not as effective. Two shots a day raise this valley and allow higher amounts of insulin at different times. My son needed more at night and less in the afternoon. Splitting the dose allowed this to happen.

We never split lantus, so no words of wisdom there - sorry :rolleyes: We do, however have Tricare, and there are a few people at our endo (Portsmouth Naval) who have the pod. I think a lot of is has to do with how hard your endo is willing to fight, or how convincing he or she is. Luckily, ours is GREAT. I really hope it all works out for you!!

I gave her the extra 1 unit last night and she stayed in the 200's all night. At 9 pm she was 241, I gave her 3 units on novolog. midnight she was 296, she got 2 units novo , 3 am she was 226, she got 2 units novo , and 6 am she was 247 and she got 2 units novo plus 14.5 of Lantus. Whenever we do a Lantus change it takes 3 days to see a full change here too. It will take a while to get the balance right. :cwds:

I found the balance! Its 14.5 units in the morning and 1.5 at night, she only had two highs yesterday (one in the 200's and one in the 300's), all other checks in the 24 hr period were IN RANGE.......:D I should be knocking on wood, cause when you say something, the shoe drops right? :cool: It just feels good to breathe for a change.

We have been split dosing Lantus for a few months now. She does well with it, except it usually causes her to be low in the mornings (50s-70s), esp. since she usually doesn't want a bedtime snack.
