I usually buy these from a woman that used to sell on ebay but i no longer have her e-mail address. Maybe someone on here had purchased from her in the past and has the information. Her name is Gillian.

I don't know her, but we buy ours from Angel Bear Pump Stuff. Jayden loves her pouch - so much so that we just ordered another one in a more neutral color.

I made one for my son over the weekend, not hard to do at all... no pattern. I just traced the size of the pump, and added for my seem allowence. I also did a clear front for butten pushing, you can get clear vynal at any craft store in the fabric department. It also has a flap to protect the vynal as long as I can. I used a pair of old jeans, cost, so minimal... maybe a buck. two at the most.

If you do a search on ebay, there a quite alot of sellers that have pump packs and stuff. I saw some when I was looking for cases this week.

thanks everyone for the replies. I went with insulinpumppacks4you but the only thing I didnt like were the belts like you mentioned.
Its too bad I cant find the person who I originally bought these belts from. She was very reasonable around $17 for the pouch & belt and the belt was one of the nice stretchy ones with matching fabric. If i come across it somehow I will post the information. Alot of the ones of e-bay i can tell are very cheap and dont use the double fabric or padding for protection.
