St. Paul, Minnesota, March 10, 2008 - Smiths Medical MD, Inc. today issued a voluntary recall of specific serial numbers of the Deltec CozmoŽ Insulin Pump.
Smiths Medical MD, Inc. today issued a voluntary recall of specific serial numbers (see below) of the Deltec CozmoŽ Insulin Pump.
Only 1023 recently manufactured pumps have been affected by this recall. These pumps were manufactured between November 2007 and January 2008. We have only been notified of 2 occurrences by customers, none of which have required any medical intervention
The reason for this recall is that the pumps may have an issue that affects how the motor operates and may cause an over-delivery of insulin which could result in injury to the user. Smiths Medical will be replacing all affected pumps.
After conducting a thorough investigation, Smiths Medical has concluded that the root cause of the motor issue was a manufacturing error with its component supplier. Through process control and process validation, this issue has now been corrected.
We will be notifying customers by mailing the recall letter via regular US Post and by telephoning customers. The Food and Drug Administration (FDA) has been informed of this action.

Interesting coincidence - my daughter's Cozmo pump had to be replaced last wednesday due to motor issues - SN not on this list however.....I had the pump in my hands in less than 8 hours after talking to customer service!

Carson's at school and I'm just dying to know what his serial number is on his pump!!! He had some very very unexpected and still unexplained lows a few weeks ago and it was quite scary. I thought it was due to using a new infusion site but he's still using the new set and his numbers seem to be going back to normal.

I might have to call Smith's today and see if his is one of the problem pumps. His was manufactured during that time frame I think.

Oops -- I didn't look at the dates close enough -- it's for pump made just recently 11/07 to 1/08 -- well, I guess we still don't know what his lows were from!!

jesse was hoping his scratched cozmo was on the recall list so he could get a brand new one.. but it wasnt, ;)

We were wondering about that with Danielle too. Some scary lows. Twice her cartridge has been empty, but not this third time. It does make you wonder though. Danielle's is one of the refurbished ones, but we got it before then.
