Rick - I have been told to ask for your help as you are amazing at helping people get their CGMs covered. We have a 3 yr old, dx at age 1 and have had the Guardian for a year now - sooooo expensive and our insurance denied coverage. We could NOT keep her in range without our CGM. Help! Thank you so much.

but I don't think that I have the skills to tackle an appeal in an "off-label" usage scenario (Jamie's under the minimum FDA-approved age of 7 years, your fight will be too tough for me).
OTOH, I'm going to be posting a bunch of de-identifed "appeal letters" and insruance company denials and policy statements in the next few weeks, please go ahead and copy anything you can use, and present any of my useful phrases AS YOUR OWN without attribution.
That goes for everybody. ;)
