This is what our nutitionist told us. Don't pay attention to the net carbs, rather take the total amount of carbs, subtract grams of fiber ONLY IF 5g or MORE, and subtract HALF of the sugar alcohols. They said the net carbs thing is misleading. Hope this helps.

WE were told to just look at the total carbs and divide it by our ratio! Not to look at any thing else cause it didn't matter.

The only thing that seems to throw off our insulin ratios sometimes is fruit and only some fruits. Blueberries seem to need a bit less insulin. Of couse YMMV;)

We were also told that on products with NET Carbs, (like atkins bars), take the total carbs minus the fiber. It usually seems right on when I bolus for the reduced amount of carbs, but the "net carbs" is not accurate for a diabetic.

We too take total number of carbs minus fiber IF it is 5 grams of more fiber. We usually don't adjust for sugar alcohols unless I can track in my history log that a certain product had a certain effect on MIa's sugars (like she went low thereafter). Has worked pretty good so far.

Our dietician advised to take the carbs and subtract dietary fiber. If there is sugar alcohol, then divide that in 1/2 and also subtract from carbs. Beware, some sugar alchohols act differently than others and may cause a spike.
