Well things WERE going well..now Samantha has a head cold and has been running higher bg's..she upped her basal, but now 2 hours after eating(restaurant meal..we were bad) She is 325. She did a correction on our own because we were not taught to do it..now what? Wait an hour to see if it goes down? If not, do shot..at one point would we change the site??
Thanks, rose and Samantha

I would wait an hour or two and see if she is coming down, if not then I would call your endo to make sure you do what they want. If it were me I would do a shot at the 2 hours if still high and if she is still rising at 2 hours after that I would change the site. I now when Hailey had a cold we had to increase her basal to 150% for the duration of the cold.

Make sure to check for ketones often.

I'm not sure how old Samantha is, but for my 12 year old, those restaurant meals are killers! (And sadly, we eat far too many of them). We'll continue to see highs for many hours afterwards unless we do a dual wave (extended) bolus. I'd check again in another hour or two and correct again.

Is she new to pumping? What type of pump is she using? The CDE or whoever did your pump start should have set up the pump to automatically calculate correction dosages based on her sensitivity factor and "Insulin on Board".

Samantha is 15, pumping mm 522, about 1 month now. We met with the rep who hooked her up, and she did set up her correction dose..but told us not to do a correction..I have tried several times to get hold of her for more teaching. Sam has changed her basals twice, for a few hours, and she has it upped now..And today she couldn't reach me so she did a correction in school.
I know we would see these numbers but it got me nervous!!

If you wait an hour after dosing and test, and she is close to her target, say 150 for example, then she will probably go hypoglycemic. If you get a number in the mid 200's after an hour, then she may be okay. If you don't budge after an hour, and she's still in the 300's, she will probably remain high. I would test after an hour, if it's not already too late. Hopefully your pump is programmed with all the correct factors to ensure she comes down gracefully and safely. It is good to know how to do the formulas on your own, especially if you are new to this, and want to corroborate what the pump is telling you.

Get the book "Pumping Insulin" - it will teach you everything you need to know.

Sam did it right. Your Pump Rep said to do no corrections to see if your IC ratios where set correctly. However, if she can't be reached - correct!!!

You have two things going on:
Cold (I think you said that - or was it jsut the wine)
Restaurant

Then you tested after 2 hours - which means lots of active insulin still.

So, correct - if you have the Minimed, it will take of the correction amount for active insulin. If she is still climbing, after correction - by like 40 points, then it is time to change the site. If she is steady, then the site is probably ok.

Good luck!
